Thoracoscopic Stage Internal Traction Repair Reduces Time to Achieve Esophageal Continuity in Long Gap Esophageal Atresia

目的 对长间隙食管闭锁(LGOA)的处理存在争议。本研究旨在比较两家高流量中心对 LGOA 的处理方法。

方法 我们纳入了 2008 年至 2022 年间的 LGOA(A 型和 B 型)患者。收集并比较了人口统计学、手术方法和结果。

结果 研究人群涉及A中心28名患者和B中心24名患者。A中心手术入路为胸腔镜,只有一名患者的最终手术是开胸手术。在B中心,3名患者仅接受胸腔镜治疗,2名患者转为开腹手术,19名患者接受开腹手术。 A中心食管一期吻合术1例,外牵引二期食管延长术1例,多期内牵引术26例。 24 名患者实现了完全吻合:23 名患者仅使用内牵引技术,而 1 名患者需要开放式 Collis-Nissen 手术作为最终处理。 B中心一期吻合7例,延迟食管吻合8例,外牵引延长食管1例,婴儿食管胃管置换术9例。 分析的术后并发症包括:早期死亡率,由于伴随畸形为 2/28(A 中心)和 0/24(B 中心); 吻合口漏,4/26(A中心)保守治疗 - 所有患者均进行对比研究 - 0/24(B中心),1例胸腔积液,但没有常规对比研究; 复发性狭窄,13/26(A 中心)和 7/15(B 中心); 以及需要进行胃底折叠术,5/26(中心 A)和 2/15(中心 B)。 A 中心的食管连续性年龄中位数为 31 天,B 中心为 110 天。初次手术和食管吻合之间的中位时间在 A 中心为 11 天,在 B 中心为 92 天。

结论 胸腔镜内牵引技术减少了实现食管连续性的时间和食管替代的需要,同时保持了相似的早期并发症发生率。
